For your convenience we offer Virtual Consultations!

How It Works

We are sorry for not being able to take care of you in person, but that does no mean we do not care.  WE DO!  Dr. Chad wants to do everything possible within the parameters of our current environment to help move your treatment forward.  To that end we have created a way to conduct a "virtual appointment".  Submit the information of the patient of record below and Dr. Chad will reach out and let you know what he would like to do to take care of your needs.

Please include your six photos matching what you see below.  You can use two tablespoons to assist in retracting your cheeks away from your teeth.  Hold your phone or camera horizontally, with the flash on or in a room with great lighting, and get as close as you can to your teeth while still showing the whole mouth.  The more detail and clarity the better!

Again, we apologize for not being able to open our doors to you at this time, but we appreciate your understanding and efforts to care for our loved ones and our community.  Dr. Chad LOVES his patients and wants to answer your questions and concerns, and keep treatment on track as much as possible
